Patients are seen five days a week at one
of the locations. If you wish to contact or make an appointment
with one of our physicians, please call 527-6300 between the
hours of 9:00 A.M and 6:00 P.M., Monday-Friday.
When the office is closed, our phones are
forwarded to a 24 hour answering service where you may leave
a message. If you need to speak with your physician immediately,
ask that the physician be paged to return your call as soon as
possible.
Insurance and Payment
As a courtesy to our patients, we will complete
and file the necessary claim forms for reimbursement of the services
rendered by our physicians. If necessary, we will
also precertify your surgery, as this can be a very time consuming
task. In order for us to provide these services, patients
must provide us with accurate insurance information. Please
print out the Patient Information Form from this website so you
can bring the completed form to your office visit.
If you are a member of an HMO, please remember
that you must bring a referral from your primary care physician,
in order to be seen by our doctors.
Insurance companies often have 60-90
day deadlines for filing claims. If you do not provide
us with your current and accurate insurance information, a
deadline could be missed and you would be responsible for full
payment of the charges. |
